Risk Adjustment Cheat Sheet

Outcome measurement in home care is simultaneously simple and complex.  Comparing OASIS data collected at Start of Care (or Resumption of Care) with the response to the same OASIS item at Discharge in order to determine a change in the patient’s health status (i.e., improvement, stabilization, or deterioration) is easy to do.  On the other hand, a process known as ‘Risk Adjustment’ is an important – but sometimes difficult to understand – aspect of outcome analysis.  The basic principles of Risk Adjustment and Outcome Measurement are included in this tool.
