  • OASIS-C2 Cheat Sheet: Obstacles to Data Accuracy

    The OASIS-C2 dataset contains significant changes that affect the clinician’s assessment strategies and require renewed focus to ensure compliant application of guidance.  Obstacles to data accuracy associated with the OASIS-C2 version include:

    • Inadequate understanding of three new items and their importance as factors in risk-adjustment of the new IMPACT outcome “Percentage of Patients with Pressure Ulcers that are New or Worsened”;
    • Insufficient recognition of modifications in renumbered pressure ulcer and medication items due to standardization with other post-acute data collection;
    • Failure to adhere to a new uniform “look-back” period in five items collected at Transfer, Death at Home, or Discharge; and
    • Significant guidance manual changes (such as in pressure ulcer documentation).

    This Cheat Sheet describes these obstacles and will mitigate risk of inadequate or inaccurate documentation.  Take advantage of this opportunity to make sure your clinical team masters the many challenges introduced in OASIS-C2.

  • Orientation Manual - Home Health

    The Home Health Orientation manual was created to orient new employees to the different facets of Home Health care.  Each chapter includes information on a different topic and concludes with a post test for the trainee.  There is an answer key for each test for the trainer to grade the trainee’s progress, and the test can then be placed in the employee file for future proof of education and training.  The Orientation Manual covers basics such as Advance Directives, Compliance and Documentation, to more in depth topics like Home Care 101, Oasis Assessment, Documentation and Audit, Performance Improvement, Oasis Start of Care Admission, and Public Reporting. This is both a great tool for providing both employee training, and a quick way to offer hour long in-services for weekly (or monthly) staff meetings.
